I believe the 话外音 in a play is definitely "narrator", and once i joined a play where my teacher said so, too.

旁白is narrator, i totally agree.

I think it was because your teacher was refering to a person of the play, whose job was to do 话外音, and that was why she used the word "narrator". But I don't think 话外音 is a person, it sounds more like a part of the structure of a playwright to me.

Is that a rhetorical question?

In my opinion, the relationship between "话外音" and "旁白" is very similar to "teaching" and "teacher", and hopefully people can understand the rest of it.
